I think you get the myth banner from Myth Adventure quest given to you by Cyrus Drake when you are level seven. You buy the myth plaque from Aubert QuickHammer in the Shopping District. My brother says you get the myth shield from the quest The Sixth School given to you by Merle Ambrose once you have defeated Lord Nightshade. You also get a book but I'm not sure when you get that. And that is all I know.
