The has happened to me twice, someone else finished before me and the people and items where not there. Both times in Grizzlehime dungeons. So, have to reset the dungeon.

Altho I am not quite sure what you mean here I will try to guess. You port into a battle in progress? They finish the fight before you are active on the pad making the monsters and item if any to pick up only available to the ones who defeated the monsters. The only other guess is, I have had a few quests where I have to find the cave first, if I have ported into the battle with out going to the door first to have finding complete it did not count for me.
