When I'm in the Pet Pavilion, Ravenwood, or Mirkholm Keep and teleport sometimes I disappear and cant log out or leave . Does this happen to anyone else?

It could be a corrupted file. Have you tried uninstalling and reinstalling the game?

just hit the recall (go to commons, go to oasis, ) icon. once it teleports you there then you'll be fine.
