I am a legendary storm and just got Insane Bolt. I understand it only has a 20% chance of back-fire, but since it is a storm spell, does my 26% gear accuracy add to that in some way?

Not that I'm aware of, no. Accuracy (as I understand it) only affects whether or not it is successfully cast.
