Well I don't know what level comes after private, but to level up do you have to buy gear or do you have to win a bunch of ranked battles? I am level 50 so I think it's time I level up.

In rank, you gain rank(s) which go: Private, Corporal, Sergeant, Veteran, Knight, Captain, Commander, and Warlord. These you get by winning ranked matches. Now after that there is gear that you can get with tickets that you get from playing ranked matches. The gear is very good. To win in pvp though you have to find some strategies that work for you. If you get to Warlord the level 50+ gear from Diego is excellent.
You just play. Your rank is based on your wins and losses. If you are going to PvP hard and make it a big commitment, I would recommend level 60. Just a piece of advice. But diego's glendemming gear is fantastico!
PvP ranks in order: Private: (0-524, you start at 500 rank.) Corporal: (525-549.) Sergeant: 550-559) Veteran: 600-649 Knight: (Unsure about this but I believe 650-699 or 600-699, I am guessing 650 though.)
