In the quest, in Khrysalis, where you have to talk to Tymen Whiteflame and defeat him he spawns as a boss before you can even get the chance to continue the quest and THEN defeat him as the quest states. I don't know if this is a bug or it's purposely supposed to happen but it has happened to me on many occasions.
